Procedure(s) for Reporting a Work Related Incident or Injury 

Employees of the District who suffer an injury/accident while on the job must file a report with their supervisor and a school nurse immediately — not to exceed 24 hours.

Procedures for Reporting a Work Related Injury:

  • You must immediately report the incident or injury to your supervisor and/or school nurse
  • Your Supervisor and/or school nurse will complete a supervisor's report of injury and forward a copy to the Human Resources Department
  • If needed, an Authorization for Treatment will be provided to the injured employee for treatment
  • Your supervisor and/or school nurse will direct you to the district's occupational health clinic for medical treatment if needed
  • Injuries requiring immediate emergency attention shall be referred to nearest hospital emergency room or call 911
  • If you are released to regular duty, you must provide your supervisor and/or school nurse and Human Resources Department with a copy of the release
  • If you are released with restrictions, you must immediately notify your supervisor and Human Resources Department prior to returning to work
  • If you are released with restrictions, restricted, light duty work may be offered by the Agency, prior to your return to work; never place yourself or co-workers at risk by performing duties that you have been restricted from performing
